Your First Visit

Adam Brown, DDS welcomes new patients. Whether you have just moved to town, are transferring your care from another practice, need to establish care with a dentist, or find yourself suddenly in need of dental service, we look forward to partnering with you for your oral health. Your first visit will be tailored to meet your specific needs. Our services include routine dental care and procedures such as cleanings, tooth extraction and root canals, but we also offer comprehensive cosmetic and restorative dentistry services including whitening, veneers, dental implants, bridges, and dentures. We treat patients of all ages. 

Adam Brown DDS Monroe Patient ServicesIf you have specific concerns about your oral health, we invite you to ask questions. If you have any fear or anxiety about dental care, please let us know. Our aim, from front desk to dental chair and back, is to make you as comfortable as possible and perform our work with a focus on quality and efficiency. We will discuss with you any problems and work together to develop a plan to achieve a healthy, beautiful smile.

Please note that all new patients are required to fill out new patient forms. We ask that you please review and complete these forms prior to your appointment. You may bring the completed forms (printed and signed) with you on the day of your appointment, fax them to us at 704.289.9104, or mail them at least 5 days before your appointment to New Patient Forms, Adam Brown, DDS, 704 South Sutherland Avenue, Monroe, N.C. 28112. If your forms have not arrived by the time of your visit, you will be required to complete them again prior to your appointment.

New Patient Forms

Our new patient forms collect important information about you and your overall health.

Oral health can impact certain diseases and certain diseases can impact oral health. For example, diabetes reduces the body’s resistance to infection and puts gums at risk. According to the Mayo Clinic, gum disease appears to be more frequent and severe among people who have diabetes. Always tell your dentist if you’re taking any medications or have had any changes in your overall health — especially if you’ve had any recent illnesses or you have a chronic condition.

The forms include patient information, our financial policy, and our privacy and disclosure form. Signing the patient information form authorizes your insurance company to pay us their designated portion of fees for services provided. Our financial policy notes that all co-payments are due at the time of treatment. Payments due must be paid in cash, approved check, credit card, or CareCredit. Our privacy and disclosure form is legally required and outlines how and to whom your health information may be released under HIPAA

Insurance Information

Adam Brown, DDS accepts most major insurance plans. We recommend that you ask one of our Insurance Assistants for specific information about your policy before initiating treatment. We are always adding more providers to our service list so there’s a very good chance that your insurance will be accepted!
