Patient Forms

All new patients are required to fill out new patient forms. We ask that you please review and complete these forms prior to your appointment. You may bring the completed forms (printed and signed) with you on the day of your appointment, fax them to us at 704.289.9104, or mail them at least 5 days before your appointment to New Patient Forms, Adam Brown, DDS, 704 South Sutherland Avenue, Monroe, N.C. 28112. If your forms have not arrived by the time of your visit, you will be required to complete them again prior to your appointment. 

As a reminder, all co-payments are due at the time of treatment. Payments due must be paid in cash, approved check, credit card, or CareCredit. As a courtesy, we are happy to file your insurance claims for you. If you would like to discuss insurance and payments before any appointment, please call us at 704.289.9519.
